Summary Of MYHEALTHCARD™ Plan Packages

There are sixteen (16) packages offered by MYHEALTHCARD™ Health Screening platform. The MYHEALTHCARD™ platform includes the provision of various packages offered to the public (individual and corporate). The packages are based on two types of testing/screening model:

1. Standard

  • This will include an annual license for the MYHEALTHCARDTM application which gives the twelve (12) months validity to the application and the period for the tests to be done by the user
  • The tests are done every three (3) months and covers all the test panels
  • The user will also receive a minimum of one (1) report depending on the type of package from the doctor/general practitioner that provides the overall review of the tests taken
  • For some on these packages, a home visit for the screening of the tests is provided by their registered SSLC
  • Standard package also includes a Standard Diabetic which includes the patient management application (MyDiabetesMyWayTM) with its host of features and also based on a one (1) year validity. Same tests results and data are used by the MyDiabetesMyWayTM application.

2. Comprehensive

  • This will include an annual license for the -MYHEALTHCARD™ application which gives the twelve (12) months validity to the application and the period for the tests to be done by the user
  • The tests are done every month and covers all the test panels
  • The user will also receive a minimum of one (1) report depending on the type of package from the doctor/general practitioner that provides the overall review of the tests taken
  • For some on these packages, a home visit for the screening of the tests is provided by their registered SSLC
  • Comprehensive package also includes a Comprehensive Diabetic which includes the patient management application (MyDiabetesMyWayTM) with its host of features and also based on a one (1) year validity. Same tests results and data are used by the MyDiabetesMyWayTM application.

Features of the account:

This is a single user for the account and has access to his or her own Patient Module and all the functions provided. All individual accounts will require consent from the account holder to allow for the use and access of the screening data as well as all other legal requirements.

The package includes:

  1. One standard and HbA1c tests
  2. One assessment report by a Doctor/General Practitioner

The package is valid for one (1) year.
The package provides one (1) account access to the MyHealthcard™ web account and the dashboard / functions of the application.

Features of the account:

Features of the account:

This is a single user for the account and has access to his or her own Patient Module and all the functions provided. All individual accounts will require consent from the account holder to allow for the use and access of the screening data as well as all other legal requirements.

The Standard Diabetic includes the patient management application (MyDiabetesMyWay™) with its host of clinical management features. Same tests results and data are used by the MDMW application.

The package includes:

  1. One standard and HbA1c tests
  2. One assessment report by a Doctor/General Practitioner

The package is valid for one (1) year.
The package provides one (1) account access to the MyHealthcard™ web account and the dashboard / functions of the application as well as the MyDiabetesMyWay™ patient web account for the management of diabetes.

Features of the account:

This is a single user for the account and has access to his or her own Patient Module and all the functions provided. All individual accounts will require consent from the account holder to allow for the use and access of the screening data as well as all other legal requirements.

The Standard Diabetic includes the patient management application (MyDiabetesMyWay™) with its host of clinical management features. Same tests results and data are used by the MDMW application.

The package includes:

  1. One standard and HbA1c tests
  2. One SARS-CoV-2 (COVID-19) Rapid Test Kit (Antigen)
  3. One assessment report by a Doctor/General Practitioner

The package is valid for one (1) year.
The package provides one (1) account access to the MyHealthcard™ web account and the dashboard / functions of the application as well as the MyDiabetesMyWay™ patient web account for the management of diabetes.

DEPENDENT

Features of the account:

This is an account where an individual set up for his or her single/one (1) family member who is above 60-years old who is not able to manage their screening and also have access to the application Here, the main account user (who may or may not have their own individual user account) will be able to manage and control all the functions of his dependent’s Patient Module and all the functions provided. This account is only for a single dependent, and if there is more than one (1) dependent, multiple dependent accounts will have to be opened. In this type of account, registration will include the requirement of a pre-signed consent form provided by the dependent (and clearly establishing the relationship with the individual main account user) with the authority given to the individual (main account user) to manage the application and access of the screening data that is stored based on the tests taken.

The package includes:

  1. One standard and HbA1c tests
  2. One assessment report by a Doctor/General Practitioner
  3. Test is to be done at home for the dependent

The package is valid for one (1) year.
The package provides one (1) account access to the MyHealthcard™ web account and the dashboard / functions of the application.

Features of the account:

FAMILY

Features of the account:

This is an account where an individual sets up for his or her direct family members (spouse and up to four children below the age of 18). All user accounts will require consent from each account holder/user to allow for the use and access of the screening data as well as all other legal requirements. Those below 18-years old will require parental approval/consent. Here, the main account user (who will not have their own individual user account) will be able to jointly with his spouse (also have a main user account with full access as well) to manage and control all the functions of the Patient Module for themselves and their registered children and all the functions provided. The main account users can add additional accounts for other family members including extended family members by adding (purchasing) additional accounts to the main account. All added adult accounts will have their own full access and their results shared under the family account, while additional children will be controlled by the main account users. In this type of account for those added as well as those below the age of 18, registration will include the requirement of a pre-signed consent form provided by the additional users (family) including those below 18-years of age with either a parent or guardian consent with the authority given to the individual (main account user) to manage the application and access of the screening data that is stored based on the tests taken.

The package includes:

  1. Two (2) standard and HbA1c tests for two (2) adults
  2. Two (2) standard tests (blood and urine panel) for four (4) children
  3. One assessment report by a Doctor/General Practitioner for each adult and a combined report for the four (4) children
  4. Test is to be done at home for the family

The package is valid for one (1) year.
The package provides one (1) account access to the MyHealthcard™ web account and the dashboard / functions of the application.

Features of the account:

This is an account where an individual sets up for his or her direct family members (spouse and up to four children below the age of 18). All user accounts will require consent from each account holder/user to allow for the use and access of the screening data as well as all other legal requirements. Those below 18-years old will require parental approval/consent. Here, the main account user (who will not have their own individual user account) will be able to jointly with his spouse (also have a main user account with full access as well) to manage and control all the functions of the Patient Module for themselves and their registered children and all the functions provided. The main account users can add additional accounts for other family members including extended family members by adding (purchasing) additional accounts to the main account. All added adult accounts will have their own full access and their results shared under the family account, while additional children will be controlled by the main account users. In this type of account for those added as well as those below the age of 18, registration will include the requirement of a pre-signed consent form provided by the additional users (family) including those below 18-years of age with either a parent or guardian consent with the authority given to the individual (main account user) to manage the application and access of the screening data that is stored based on the tests taken.

PACKAGE PLAN DETAILS (CORPORATE)

CORPORATE

Features of the account:

This account is provided to companies, institutions or any organizations that are registered with Suruhanjaya Syarikat Malaysia (SSM), Suruhanjaya Koperasi Malaysia (SKM), Jabatan Pendaftaran Pertubuhan Malaysia (ROS) or any government ministries or agencies. A corporate user also includes legally appointed public servants or officials (with all the required qualifying documents and consents as detailed below) including Member of Parliament (MP), State Representatives (ADUN), Village Head or Leader, or any individual that has been given the legal authority. For this, it includes:

  1. This is an account where a nominated representative (main account user) of the entity (companies, institutions or any registered organization) setups and provisions the screening package for its staff or members within the entity. By requirement, any nominated individual will require a board resolution that is fully certified by the company secretary to nominate the company individual to act as the nominated main user account as well as all company statutory documents for approval before the account is fully activated.
  2. All user accounts will require consent from each account holder/user to allow for the use and access of the screening data as well as all other legal requirements.
  3. Here, the main account user will be able to setup the staff account and decide on the packages and screening tests taken by each staff, with the individual staff able to manage and control all the functions of the Patient Module for themselves and all the functions provided.
  4. The main account users can add additional accounts for other additional staff on top of the package number of available accounts by adding (purchasing) additional accounts to the main account. All added adult accounts will have their own full access and their results shared under the entity account.
  5. In this type of account for additional users, registration will include the requirement of a pre-signed consent form provided by the additional users with the authority given to the nominated representative (main account user) to manage the application and the data that is stored based on the tests taken.
  6. For this type of user, the screening tests are done at the site of the entity by their registered SSLC based on a schedule set with the centre that the entity is registered with.
  7. Special approvals and conditions apply to circumstances where the individual user is not able to manage the screening/application including those who are immobilized or handicapped where consent is required from a family member or legal guardian that will allow a third-party individual to manage for the individual (for example in the setting of an aged facility).

The package covers up to five (5) staff accounts.

The package includes:

  1. One (1) standard and HbA1c tests for each staff (adult)
  2. One (1) SARS-CoV-2 (COVID-19) Rapid Test Kit (Antigen) for each staff (adult)
  3. One assessment report by a Doctor/General Practitioner for each staff (adult)
  4. Test is to be done at the office site for all staff (adult)

The package is valid for one (1) year.
The package provides one (1) account access to the MyHealthcard™ web account and the dashboard / functions of the application.
